Against the Minority Pawn Attack

How to defend against the well-known, frequently used minority pawn attack? (White's attack in Queen's Gambit with a- and b-pawns against Black's a-, b- and c-pawns.)

One method is to exchange Knights at e4 and then attempt an attack with pawns in center and King side. Position after 7. Bh4 is typical. Can Black now move Ke4 safely? After exchange, pawn e4 is not easy to defend. White's King, however, is two moves away from castling, and this allows Black develop a strong attack if White does not play with care.
